Helpful guidelines for filing a Claim
Prior to filing a claim against the City of Hemet, please review the following guidelines:
- Claims for death or injury to person or to personal property must be filed no later than six months after the occurrence.
- Claims for damage to real property must be filed no later than one year after occurrence.
- The presentation of a false claim is a felony (Penal Code Section 72; Insurance Code Section 556.1).
Submitting a Claim for Damage or Injury
To orderly and efficiently process your claim, please print or type clearly all of the information requested on the form. Incomplete claim forms may be returned to you as insufficient. Be sure your claim is against the City of Hemet, not another public entity. Providing photos and other pertinent material can help with the claims process. Completed originally signed claim forms may be mailed or personally delivered to:

Please fill out the claim form completely. Additional sheets may be attached if more space is needed. Missing information may delay the processing of your claim. The original, with wet signature, and one identical copy of this claim form, together with a copy of all attachments, are to be filed with the City of Hemet. Please retain one copy for your records. Claim Processing
Once received, the matter will be referred to the Finance Department and a third-party for review and response.
